To refill a prescription:
Please send an email to victoria@sageclinic.com and specify the items, the prescribing doctor, payment method, and whether you would like to pick up the order or have it shipped to you.
Missed Appointment and Cancellation Policy:
As a courtesy, we will attempt to contact patients by email or phone to confirm scheduled appointments ahead of time. Please note we require a minimum of 24 hours notice (business days) for appointment cancellations or changes. If a minimum of 24 hours notice (business days) is not provided a cancellation fee may be applied.
Please note that the cost of HST, laboratory testing, and medicinary items is additional and is not included in the above fees. Payment is due at the time services are rendered.
