EXTENDED HEALTH PLAN COVERAGE Increasing numbers of employers are providing Extended Health Plan benefits to their employees. Most of these plans cover a percentage of Sage Clinic's services (naturopathic medicine and acupuncture) up to a certain annual limit. Some plans have a limit on their reimbursement per office visit. Others will also provide coverage for naturopathic and Chinese herbal prescriptions. If you have an Extended Health Plan, please contact your
benefits department or plan provider for information about the type and amount of
services and prescriptions covered. Sage Clinic will work cooperatively
with you to ensure you receive the maximum health benefit
